Summary of the Best Practice: ResCare utilizes this tool to receive feedback from individuals they serve using open ended questions.
Practice: This best practice is an all inclusive review form for the provider to utilize to receive feedback from individuals receiving services and supports. The form contains open ended questions that focus on the areas of personal goals, choices, social inclusion, relationships, rights, dignity and respect, health, environment, security and overall satisfaction with services and supports. The best aspect of this document is that it is a “single document” that encompasses all areas of an individual’s interests and needs. This document also includes a summary section at the end to identify an individual’s needs to improve their services and supports. This document is definitely driven by the individual!
Who Benefits?The individual is being afforded the opportunity to give their feedback in a non- restrictive manner. The provider benefits by acquiring information from the individuals they serve in order to provide person centered services as well as trending the results from these assessments to improve their overall service delivery system.
