Summary of the Best Practice: 
The provider uses a form called Individualized Service Plan Review/Addendum to review and document an individual’s progress or lack of progress on their goals and the reasons why.  Based upon this evaluation, if there are changes needing to be made to supports to assist the person in meeting their goals, and/or changes to objectives and/or goals this documents what those changes are.  At the bottom it also identifies whether there are any critical medical issues occurring which may alter supports, goals and objectives. 

The only recommendation to this form is to include a space for the individual receiving services to sign as well.  By doing so will demonstrate the person’s involvement in the review of progress and the decision making to change supports, objectives or goals. 

Who Benefits and How? Provider benefits from looking at the effectiveness of their supports and services to helping a person meet their goal/objective and providing an opportunity to make changes according to the person’s preferences.  The individual benefits from being a part of directing their supports and services.

Provider uses a comprehensive form entitled, “Assessment of Rights for Persons Receiving Services,” which provides a means to obtain a detailed understanding of each individual’s ability to understand and exercise their rights; to identify the needed supports, including any restrictions; and to assist the individual with obtaining this information.

The individual receiving services benefits due to the in-depth review this assessment provides in reviewing their rights in many realms of their life.

How?
The evaluation is completed annually, or as needed, to determine the rights each individual receives in their everyday life. The individual’s ability to understand and exercise their rights, and identify the supports that are needed, are identified and these issues are included in future planning.

Their dedication to assisting individuals has resulted in making a request that an agreement concerning the visitation, medical or medication needs of the individuals are met when not in their personal home. The form “Visiting Away Agreement” gives clear instruction to the responsible party in writing as part of the health and safety needs of the individuals they serve.

Who Benefits and How? Individual benefits, as their health and safety needs are clear while visiting family and friends. Family and friends benefit, as they become more comfortable with assisting individuals as the medical/medication needs are in writing versus verbal instructions. Henry’s Personal Care Home staff benefit, as they have a clear understanding as to the visitation timeline to ensure the proper amounts of medication is dispensed to the individual prior to leaving the home and have a written understanding as to the individuals departure/arrival to their personal home.

Summary of the Best Practice: Henry’s PCH strived to keep individuals informed about their financial status. They have developed a tracking system form “Consumer Expense Tracking Sheet” that affords everyone on the individuals team to have a financial snapshot.

Who Benefits? Individuals benefit, from this tracking sheet by becoming aware of their money management and independent with their finances. Henry’s Personal Care home benefits, as they become transparent in the accounting of financial matters for the individuals they serve. Support Coordination benefits, as they can utilize the accounting method to quickly identify the spending patterns for their reporting processes during a visit to the home.

How? All team members who utilize the tracking sheet come to a better realization as to the financial status of the individuals they serve.

Summary of the Best Practice: Independent Enterprises has an “Activity Capability Release Form” that is signed by the physician. It indicates the level of capabilities an individual has in following areas such as level of walking, health and safety support, program identified health concerns (completed by I&E staff) for the individuals, any special instructions related to health concerns and any restrictions. These restrictions include ground level activities, outdoor activities (i.e. cannot use feet for repetitive movement or no swimming/water activities), etc.

Who Benefits? It is of benefit to the individuals, Independent Enterprise staff and Support Coordination Agencies

How? For the individuals, it supports their capabilities and limitations. For Independent Enterprises Staff, it allows them to advocate on behalf of the individuals served concerning their health care and allows them to gain a better understanding of how an individual should be supported along with including this information in the HRST. For Support Coordination Agencies, it allows them to include these limitations in the individuals ISP and SIS. This also affords the opportunity for the support coordinator to ensure that action plans are written according to the individual’s capabilities.

Summary of the Best Practice: Georgia Pines has developed a “medical tracking Sheet” that assists in the ongoing quality assurance concerning individuals appointments and medical needs. This tracking sheet provides a timeline of all medical appointments for the individual as well as reason for appointments, results of the appointment and whatever follow up is needed or when the next appointment is scheduled.

Who Benefits and How? Individual’s benefits as they have a tracking of all their medical appointments that ensures that their medical needs are not overlooked and kept current. Georgia Pine Staff benefit, as they have a running log of all medical services being assisted by staff that serve the individual and can use this tracking sheet to inform emergency personnel if an event occurs in an individual life while in their services. Support Coordination can benefit, the form gives a clear snapshot as to the individual’s current healthcare status and needs of the person in the future.

There was even a section for dreams and visions, photographs, and much more, if desired.

Summary of the Best Practice: The provider did an Easter Egg hunt in the community.  They first contacted community organizations to participate and for those that were willing, the provider placed Easter Eggs at those locations.  The provider put together small groups of individuals who wanted to participate. Each group was given clues to help them find the community organization in order to find the eggs.

Practice: This best practice is an all inclusive review form for the provider to utilize to receive feedback from individuals receiving services and supports. The form contains open ended questions that focus on the areas of personal goals, choices, social inclusion, relationships, rights, dignity and respect, health, environment, security and overall satisfaction with services and supports. The best aspect of this document is that it is a “single document” that encompasses all areas of an individual’s interests and needs. This document also includes a summary section at the end to identify an individual’s needs to improve their services and supports. This document is definitely driven by the individual!

The individual is being afforded the opportunity to give their feedback in a non- restrictive manner. The provider benefits by acquiring information from the individuals they serve in order to provide person centered services as well as trending the results from these assessments to improve their overall service delivery system.

The provider benefits from having a clear understanding of how the individual expresses pain and discomfort and is able to update this information in the individual’s file documentation.

Practice: Provider has developed a comprehensive rights training booklet (see link below) that is utilized within their service delivery system as an on-going training tool. It affords persons with all communication and learning styles to explore their rights and develop skills necessary to implement and take responsibility in exercising the rights that matter most to them.

Who Benefits?
The person receiving services benefits from learning more about their rights.  The direct support professional benefits from learning from the person what rights are important to them and whether the person needs additional support in learning even more about their rights as a citizen.

Practice: Provider affords an opportunity for each individual within their program to easily understand their rights.  Provider has created a nine-page publication, where each "right" is written in large font, with multiple colored graphics, explaining each right they are entitled to receive.  When staff is reviewing the rights to individuals receiving services, they combine a verbal, with visual, to ease the individual in better understanding their rights.

The person receiving services benefits because it gives them a sense of better understanding their rights and what to do if they feel they are not being provided this opportunity.
