How do I get my organization's information on the Provider Public Reporting website - www.georgiaddproviders.org?

If you are a provider of either NOW/COMP Waiver and/or Grant In Aid services for individuals with intellectual or developmental disabilities, please follow the steps below to submit data needed to get your organization's information on the Provider Public Reporting website:

Step 1: Print and completely fill out the Zoomerang Pre-survey form [pdf].

Step 2: Now that you have completed the Zoomerang Pre-survey form, you are ready to complete the Online Zoomerang Survey (see Zoomerang link below)

REMEMBER! Once you start the Online Zoomerang Survey you have to complete the entire survey. You cannot exit out and go back in to re-enter your organization’s information. So be prepared to enter in all of the data as taken from your completed Zoomerang Pre-survey form.
